IRS Form 1099-SA were mailed on 01/31/2017 and are available to you online. This form reflects the total amount spent from your HSA during the 2016 calendar year (January 1 through December 31).

NOTE: If you did not spend any of your HSA balance during the 2016 calendar year, you will not receive a 1099-SA.Your distribution amount will simply be 0.

IRS Form 5498-SA will be mailed out and available to you online at the end of May. This form indicates your total contributions from the 2016 tax year. 2016 contributions may be made up until the federal tax-filing deadline (usually April 15) of 2017.

NOTE: Your 5498-SA is informative only and does not need to be filed with your taxes. Your contribution totals are also located in Box 12W of your Form W-2 you will receive from your employer. Any manual contributions made apart from payroll deductions will need to be added to your contribution total.
